For returning Scouts, please provide the following:

- A check for annual dues in the amount of $175 made out to “Cub Scout Pack 30″

- Updated copies of your health insurance and car insurance, if they have changed since last year

- A signed copy of the Health Consent Form for their Scout. We must collect the Health Questionnaire each year, even if there are no changes to your Scout’s information since the previous year. The document is a fillable PDF, so you can simply keep a completed copy on your computer and update it each year with the current date and signature.
