Application Forms – New Members

New members must complete the following forms:

1. Pack 30 Enrollment Form
2. Youth application – On the Scouting.org page, click on the Youth Application link to download the PDF, review all pages, then fill out page 3.
3. Adult application – On the Scouting.org page, click on the Adult Application link to download the PDF, review all pages, then fill out pages 3 & 9.
4. Health Consent Form (PDF download)  Please complete all 3 pages.

You will also need to submit the following documentation:

1. Photocopy of your Scout’s up‐to‐date health insurance card
2. Proof of up‐to-date vehicle insurance (so you can drive others when carpooling for Pack and Den outings)
3. Check for $175 annual dues
   • Please make check payable to “Cub Scout Pack 30”
   • or submit proof of payment via PayPal

You may submit your forms and dues check to our Membership Chair at the next regularly scheduled pack meeting.
